RATES AND INSURANCE

Insurance 

All BCBS plans are accepted including BCBS Medicare Advantage plans.  

All Aetna plans are accepted.

All BCN plans are accepted.

*BC Complete is not currently accepted.

Your individual amount owed for sessions will vary based on your plan.  Calling your insurance is the best way to find this out. Details on what to ask your insurance rep are below.


For those with insurances I am not paneled with, you may still receive reimbursement from your insurance company's "out-of-network" or "non-participating provider" benefit.  If you would like to seek reimbursement from your insurance company, I will provide you with an electronic receipt known as a "superbill" to submit for coverage as often as you require. 

 

Below, I have listed questions which you should ask your insurance provider regarding in-network or out-of-network benefits. You should call the phone number on the back of your card. 

All of the following questions are appropriate to ask your insurance provider for those seeking out-of-network services.

In-network benefits for BCBS, BCN and United members should  focus on questions with the * (see below).  

If your insurer asks about "CPT codes", the following codes are commonly used for psychotherapy - 90791, 90837, 90834. You may also check here to understand more about deductibles, co-pays, and co-insurance. 

  • *Does my insurance cover telehealth for psychotherapy services?

  • *Is Great Lakes Teletherapy, LLC/Leah Brock, LMSW in-network for me? 

  • *What is my co-payment?

  • *What are my co-insurance costs/percentages?

  • *Is there a deductible? If so, does it renew on a calendar- or policy-year basis?

  • *How much do I have left on my deductible and what does that mean for this psychotherapy?

  • *Is authorization required, and if so, how do I get it?

  • What are my out-of-network benefits for individual outpatient psychotherapy?

  • How much will I be reimbursed for psychotherapy services provided by an out-of-network provider?

  • How do I submit claims for out-of-network coverage/reimbursement?

Rates

$150 per hour 

$80 per half-hour

Call with questions involving sliding scale. 

Payment

Credit cards are required to be kept on file and will be utilized to collect balances owed for services. Credit cards will be run in collaboration with insurance processing times and/or after sessions. Late cancellation and no-show fees apply.
